In order to keep the appointment, all patients must complete an intake form sent by email to him/her prior to the appointment. Also, all patients must wear a mask – temperature reading will be done and COVID-19 update questions will be asked before the patient is seen by the doctor – these procedures are being done to protect the health of our patients and staff.
